DOT Physicals / Medical Cards

Cimarron Chiropractic offers DOT/CDL Medical Exams to Perkins, Oklahoma and surrounding areas or for anyone just passing through.
Dr. Hershberger is a registered medical examiner on the Federal Motor Carrier Safety Administration’s National Registry of Certified Medical Examiners.  Dr. Hershberger has completed the required training and testing concerning the Federal DOT Physical Exam qualifications and standards. Dr. Hershberger also continues his education which allows the office to be the most up to date when it comes to FMCSA medical standards and changes to those standards.

Cimarron Chiropractic has partnered with over 30 local companies to provide them with their FMCSA / DOT needs, including Hiland Dairy Foods, CREC, Riggs Tree Service and Stimulation Pumping Services.

Our office routinely schedules DOT Physical Exams same day or at least within 1-2 days.  Our unparalleled convenience of little to no waiting allows drivers to be in and out of our office in a timely matter.

Our DOT Medical Exams are among the most competitive when it comes to pricing.  Our flat rate of $110 per DOT Medical Exam makes us affordable and convenient.

Cimarron Chiropractic also serves as a collection site for DOT and non-DOT Urinalysis and Breath Alcohol Tests.

What to bring:

Certain documentation and items can be brought to your examination to help accelerate the process and get you back on the road:

  • List of prescription medications, the dosage, and prescribing physician for each medication
  • If applicable, documentation for CPAP compliance or an eye exam within the last 12 months
  • If applicable, your hearing aids, eyeglasses or contacts
  • Documentation of compliance and clearance for medications or procedures that may interfere with your ability to safely operate a CMV

If you are currently taking prescription blood pressure medication, diabetic medication or using a CPAP, please note that you will only be allowed a maximum certificate of 12 months instead of the standard 24 due to the need to regularly monitor our CDL drivers. The FMCSA recently changed their stance on the use of needle-based insulin use.

Any drivers seeking a CDL/DOT medical card that use insulin to manage their diabetes MUST have the insulin form filled out by your Primary Care Physician PRIOR to coming to our office for an appointment.

The FMCSA requires all drivers to pass a vision test with a Snellen Chart Test no greater than 20/40. If you are considered to have monocular vision (one eye passes with 20/40 or better, and the other does not) you MUST have the Vision Evaluation Report filled out by an ophthalmologist or optometrist PRIOR  to coming to our office for an appointment.
